The best way to wear the Fat Boy Thin Extender is to add water based lubricant to the inside, slip onto the penis and then pull your testicles through the highly stretchy hole at the end.  Attaching around the testicles ensures the piece will not slip off particularly if using for penetration.  If using as a stroker/mastubator you may not wish to do this step.
